The key principles

Function is more than strength

Pelvic floor physiotherapy targets motor control, agility, and coordination, not just muscle size or isolated squeezing.

The pelvic floor must tighten a split-second before a cough or landing, sustain gentle support over a long walk, and fully relax for emptying. These are distinct skills, and each can be trained.

TimingAutomatic pre-contraction before pressure spikes, cough, laugh, lift, impact.
Graded controlSmall, medium, or strong contraction depending on demand, not all-or-nothing.
EnduranceSustained gentle support through standing, walking, and daily activity.
RelaxationFull release to allow unobstructed bladder and bowel emptying.

When referral makes sense

  • Urinary leakage, stress, urge, or mixed
  • Pelvic organ prolapse symptoms
  • Chronic pelvic or hip pain
  • Painful intercourse or vaginismus
  • Constipation or difficult bowel emptying
  • Pregnancy and postpartum concerns
  • Pre- or post-pelvic surgery rehabilitation
  • After prostate surgery incontinence

When to wait or investigate first

  • Active pelvic infection (UTI, PID), treat first
  • Unexplained severe or new pelvic pain
  • Active or unexplained vaginal bleeding
  • Recent pelvic surgery, surgeon clearance needed
  • Patient not yet ready or consenting
  • Unstable cardiac or neurological conditions
  • Suspected malignancy, investigate first
